Optimum Global Gem plans for expats in the Gulf and Europe
Optimum Global sells its Gem international health plan with benefit guides in US dollars, sterling and euros, and offers more than one way to underwrite your medical history. IPMIEMEA explains what Optimum publishes and how the plan fits expats in the Gulf and Europe.
- No universal annual maximum is published; the currency-specific Gem guide supplied with your quote sets the limits that apply.
- Deductible, coinsurance and copayment are different mechanisms, all defined on the schedule or benefit table you are quoted.
- Contact Optimum before planned inpatient or day-case treatment, and ideally within 24 hours of an emergency admission.
- Optimum aims to assess claims within 14 working days, a stated target rather than an observed average.
- Optimum Global is not connected to VUMI's similarly named Optimum VIP plan, so do not confuse the two.
What the Gem plan is, and what it is not
Optimum Global's international private medical insurance (IPMI) is the Gem plan, sold to individuals, families and groups living or working overseas. It is published as separate benefit guides in US dollars, pounds sterling and euros, so the currency you choose sets the guide you read and the figures that apply to you. It is not connected to VUMI's similarly named Optimum VIP plan.
The current document library does not expose a single universal limit table in page text, which is why no annual maximum is quoted here. The numeric limits come from the currency-specific guide supplied with your quote, and that guide is the authority.
| Feature | Published position |
|---|---|
| Hospital cover | Inpatient and outpatient options; planned inpatient or day-case treatment requires contact in advance |
| Cancer | Available within the selected benefit table |
| Evacuation and repatriation | Available within the selected benefit table |
| Cost sharing | Deductible, coinsurance and copayment are defined on the schedule or benefit table |
| Currency | Separate Gem benefit guides in USD, GBP and EUR |
How to read a Gem benefit guide
Because Optimum publishes by currency rather than by a single tier ladder, the sensible order is: pick the currency you earn or budget in, request that guide, and then work down three questions. Is outpatient included or an option on the configuration quoted? Which of deductible, coinsurance and copayment apply, and at what levels? And what are the cancer and evacuation benefits within the selected table? The three cost-sharing terms are different mechanisms: a deductible is paid before the plan pays, coinsurance is a percentage of each bill, and a copayment is a fixed amount per item. Our guide to inpatient versus outpatient cover covers the first question, and the EMEA cost guide covers the second.
Underwriting routes
Optimum's document library includes an individual medical questionnaire, a continued personal medical exclusions (CPME) document and a moratorium guide. That tells you the plan can be written on more than one basis: a declared-history route via the questionnaire, a moratorium route, and a CPME route for people switching from another insurer who want their existing exclusions carried across rather than being underwritten afresh.

Pre-authorisation and claims
- Members must contact Optimum Global before planned inpatient or day-case treatment.
- In an emergency, the membership guide asks for contact as soon as possible, ideally within 24 hours of admission.
- A Guarantee of Payment can allow settlement directly with the provider; other eligible claims require satisfactory proof.
- Optimum says it aims to assess claims within 14 working days. That is a stated target, not an observed average.
Using Optimum Global from the Gulf or Europe
The three-currency approach is the feature that maps most neatly onto IPMIEMEA's countries. A resident of the United Kingdom, or a British expat in the UAE or Bahrain who still budgets in pounds, can hold the sterling guide; a resident of Spain, Portugal or Germany can hold the euro guide; and a dollar-paid professional in Saudi Arabia or Qatar can hold the dollar guide. The material behind this page does not state Optimum's area-of-cover options, so confirm at quote whether the USA is included and whether any narrower area is offered; our guide to Europe regional versus worldwide cover explains what to ask.
The entity structure is also worth noting from a European perspective. The policy documentation identifies Optimum Global Insurance Company Limited, a Guernsey insurer, as issuer, and describes AXA PPP healthcare reinsurance. Optimum Global Limited is FCA-regulated for insurance mediation. For UK-linked buyers that regulatory footprint may be reassuring; for everyone it is something to confirm on the issued schedule.
Gulf residents generally need a locally compliant policy, usually arranged by the employer, and an international plan is bought as a top-up or by people the local scheme does not cover. Check the current rule where you live and read our guide to mandatory Gulf health insurance and top-up cover.
Who Optimum Global suits
A reasonable fit for someone who wants to hold a plan in sterling or euros rather than dollars, who values a choice of underwriting route including moratorium and CPME, and who is comfortable reading a currency-specific benefit guide rather than a one-page tier ladder. Groups are also within Optimum's stated customer base. Compare alternatives if you want published annual maximums in front of you before you quote, or a longer list of plan tiers. Nomi prices Optimum beside the rest of the market for your details, and Expatriate Healthcare is another three-currency insurer to read alongside it.
