Cigna Global vs Allianz Care for expats in the Gulf and Europe

Cigna Global and Allianz Care are two of the strongest international insurers, and they win on different things: Cigna on premium control and chronic-condition support, Allianz on how it underwrites your history. IPMIEMEA sets them side by side for the Gulf and Europe.

In short: Choose Cigna Global if you want to set the premium precisely with a deductible, cost share and out-of-pocket cap, value nurse-led support for an ongoing condition, and want in-network direct billing on an ID card. Choose Allianz Care if you have medical history and want to choose how it is underwritten, full medical underwriting, moratorium or switch terms, behind a core-plus-modules structure. Neither is reliably cheaper.
Key takeaways
  • Cigna's cost share runs 0%, 10%, 20% or 30% after the deductible, with an out-of-pocket maximum capping the yearly total.
  • Allianz's moratorium asks no questions but imposes a 24-month waiting period on pre-existing conditions; Cigna underwrites on disclosed history.
  • Cigna's Chronic Condition Management programme is nurse-led active support, not only claims payment, for conditions like diabetes.
  • Cigna's Close Care covers residence and nationality countries only with a US$500,000 maximum, an option Allianz does not publish.
  • Both offer worldwide and worldwide-excluding-USA areas that include the Gulf and Europe, so either can sit above a Gulf employer policy.

Where each one wins

Both are international private medical insurance (IPMI) plans for expatriates with worldwide or worldwide-excluding-USA areas. The difference is what each lets you control.

Cigna wins on control and ongoing care. Three independent levers shape the premium: a deductible, a cost share of 0%, 10%, 20% or 30% on eligible bills after it, and an out-of-pocket maximum that caps what the cost share can add up to in a year. That lets you tune the price more finely than most insurers allow, and the out-of-pocket cap keeps the downside known. On top of that, Cigna's Chronic Condition Management programme is led by specialist nurses and supports members living with pre-diabetes, diabetes, high blood pressure, musculoskeletal pain and arthritis. It is active management rather than claims payment alone.

Allianz wins on underwriting flexibility. It is one of the few insurers that lets you choose the basis: full medical underwriting (20 health questions, certainty from day one), moratorium (no questions, but a 24-month waiting period on pre-existing conditions), or CPME and switch terms when you move from another insurer. If there is anything in your history, that choice can change what is covered from the first day. Cigna also underwrites on disclosed history, and requires you to report any change in health before the start date, but it does not offer the same menu of routes.

On claims, the two differ in process rather than quality. Cigna's in-network outpatient visits can often be billed directly when you show the Cigna Healthcare ID card, and most outpatient care needs no prior authorisation. Allianz requires pre-authorisation for inpatient and high-cost treatment and pays only 80% of inpatient and 50% of other eligible benefits if you skip it; emergencies must be reported within 48 hours, and out-patient and dental claims go through the MyHealth app. The full profiles are at Cigna Global and Allianz Care.

Cigna or Allianz from the Gulf or Europe

Both insurers offer worldwide and worldwide-excluding-USA areas, and both versions include every IPMIEMEA country, so the area decision is the same with either: leave the USA out unless you expect planned treatment there. Cigna adds one option Allianz does not have in the material behind this page: Close Care, which covers your country of residence and country of nationality only, with a US$500,000 maximum. For a British national settled in Spain or Portugal who rarely travels elsewhere, that is a cheaper shape than either insurer's worldwide plan; for a Gulf-based professional who moves between the UAE, Qatar and Europe, it is too narrow. Our guide to Europe regional versus worldwide cover explains the trade-off.

In the Gulf states, residents generally need a locally compliant policy, usually arranged by the employer, and either of these plans would typically sit on top of it or serve people the local scheme does not cover. Check the current rule in your emirate or state and read our guide to mandatory Gulf health insurance and top-up cover. Where the two diverge for a Gulf expat is history: someone who has picked up a condition while abroad and now wants a plan that will follow them home to Germany or the United Kingdom will usually find Allianz's underwriting menu more useful, while someone in good health who wants to manage the premium tightly will usually find Cigna's levers more useful. Cigna's published maximums are in US dollars; Allianz's plan currency is confirmed at quote. Our guide to pre-existing conditions explains the underwriting terms both insurers use.

Which should you choose?

Choose Allianz Care if…

You have medical history and want to choose how it is underwritten, you prefer a clean core-plus-modules structure where out-patient and dental are added at the same level, and you like the backing of one of the world's largest insurers.

FAQ

Is Cigna or Allianz cheaper?
Neither is reliably cheaper. The price depends on your age, country, cover level, excess and medical history, and the same person can get very different quotes from each. That is why IPMIEMEA suggests pricing both live for your own details rather than assuming.
Which is better for pre-existing conditions?
Allianz Care is often the stronger pick because it lets you choose the underwriting basis: full medical underwriting, moratorium with a 24-month waiting period, or CPME and switch terms when moving insurer. Cigna also underwrites on your disclosed history. The best route depends on your specific conditions, so take advice.
Which has better chronic condition cover?
Cigna stands out for ongoing management through its nurse-led Chronic Condition Management programme. Allianz covers chronic conditions per plan, managed through underwriting. Whether an existing condition is covered when you join still depends on underwriting with either insurer.
Which works better as a Gulf top-up plan?
Both offer worldwide-excluding-USA cover that includes the Gulf and Europe, so either can sit above a locally compliant employer policy. Cigna's deductible and cost-share levers suit someone who wants a lean top-up for large events; Allianz's underwriting choice suits someone with history. Check the current local rule before relying on either as top-up.
